Visa application form.
Important Note: Please upload or send us electronically scans of ALL documents needed for the generation of your application form which are listed in all the requirements below;
Original passport.
The passport must be valid at least 6 months after the intended date of departure from the territory of the destination country and has at least 2 blank visa pages.
Passport-type photograph.
One digital photo. No physical photo needs to be submitted.
Enrollment Letter and JW-202 Form.
Original enrollment letter from the school in China and JW-202 Form.
Previous visas.
A copy of your last visa to China, If the visa is still valid you must submit the old passport with the old visa in it. If you are no longer in possession of your previous Chinese visa you must include a letter to the Chinese consulate or embassy explaining when and where you received your previous visa and how the passport and visa were lost.
Proof of residency.
Copy of driver's license, state issued ID, or major utility bill (Water, Gas, Electric, Sewage) for the most recent month, showing the applicant's name and current address.
